Before submitting your application, please carefully read Parade Guidelines.

We MUST receive these three forms by July 12, 2013:

• Parade Application form (submit online or print and mail form, address below)
• Indemnification Agreement form (this form MUST be signed and mailed)
• Media Information form (submit online or print and mail form, address below)

Every applicant must mail in a completed and signed Indemnification Form.
